ESSDA Application 2010 - 2011

On line registration will help with the administration for Match Secretaries.

ALL APPLICATIONS MUST BE RECEIVED BEFORE 30th JUNE 2010
LATE APPLICATIONS WILL NOT BE ACCEPTED.

Please remember to include sufficient postage if using a large envelope!

All club coaches should have appropriate qualifications and evidence should accompany all applications - photocopies of certificates. Clubs who fail to do so will not be accepted into membership of the Association.
New coaches have 6 months to achieve qualifications. Coaches named on previous applications must have these qualifications as their 6 months probationary period has expired.

Clubs joining ESSDA must also be Associate Members of SYFA.
SYFA Membership is an online process from www.syfaregistrations.co.uk

eMail addresses will be used by ESSDA to contact coaches and club officials to keep them informed of ESSDA business and may be given to SFA and/or SYFA for similar purposes.
eMail addresses will not be given to 3rd parties, but with consent will be given to other coaches.

• Application Form for Season 2010-2011 (7 a side team)
  The completed form should be printed and sent to ESSDA together with copies of Coaching and First Aid Certificates and payment of £30 per team.
  The entered details will be emailed to your Match Secretary, but the accompanying paperwork must also be completed.
• Application Form for Season 2010-2011 (4 a side club)
  The completed form should be printed and sent to ESSDA together with copies of Coaching and First Aid Certificates and payment of £20 per team.
  The entered details will be emailed to your Match Secretary, but the accompanying paperwork must also be completed.
• Monitoring Form for Season 2009-2010 Second Half
  This form can be completed on-line with the results sent directly to the Match Secretary by email.
• SYFA Registration
  The online process can be found at www.syfaregistrations.co.uk

  Please download the form, complete and send to Your Match Secretary. You may prefer to complete the form online. See above.

Permission of the East of Scotland Soccer Development Association is required before participating in any Tournament or Friendly match if the opposition is not in membership of the Association.

Permission is also required to be obtained from the Scottish Youth Football Association.

The SYFA form can be obtained from their website - www.scottishyouthfa.co.uk Go to section Member's Notice board, subsection Tournaments.

Without properly authorised permission, your club insurance may not be valid.
