A single match monitoring form can be completed on line - Monitoring Form

Submitting a monitoring form can help ESSDA / SYFA decide what teams may or may not need watching.

Please submit a form if your match went well or if went not so well. That way we can build up a picture of good/bad behaviour.

If you give poor marks, please add an explanation/comment to clarify your thoughts.
